As for 6, Dec this year, Amazon released the digital and softcover 5 days ago, as well as it's been on the shelves at Barnes & Noble.

Amazon and Barnes & Noble have the hardback and paperback version listed. Here's the Amazon info:

• Item #1427871523 is the hardcover version with 176 pages, releasing on December 13.

• Item #1427871752 is the paperback version with 176 pages, releasing December 6.

Also on Amazon is Item #B0BNV2FZCX, which has 100 pages and the word "Notebook" at the end of the title. It has a different person as the author, so I'm guessing this may be just a blank notebook to write in but it's using the same cover as the official release.
